Physical therapy is the medical discipline focused on restoration of movement and function while managing pain. Our goal is to assist our patients in returning to work, play and life through treatment in the clinic and patient education.

NXT Level Physical Therapy is dedicated to providing physical therapists with cutting-edge resources and training necessary for learning and implementing the evaluation, diagnosis, prognosis, and intervention skills necessary for evidence-based practice. We are dedicated to attracting and retaining highly accomplished professionals to provide and support personalized orthopedic, sports and industrial rehabilitation, and women’s health services in a positive, friendly, and upbeat environment. Our practice is built on the philosophy of delivering services to the patient by a professional physical therapist or physical therapy assistant with a continuum of care by the individual therapist or team from the initial patient evaluation to discharge.

You will need to bring your doctors referral (prescription), insurance cards, patient forms if you filled them out in advance, a list of current medications, a method of payment, and appropriate clothing to your first appointment. Additional items may be requested by our staff depending on your specific situation. To find out if you will need any additional items please contact our clinic.

At your first appointment you will meet your treating physical therapist. You will be taken to a private treatment room and discuss your past medical history, current problems/issues as well as your treatment goals. Your physical therapist will perform a comprehensive evaluation/examination, inform you of the findings and set your treatment goals at that time.

If you have any condition that impairs your mobility, limits your range of motion and causes pain, then physical therapy may be able to help. Our physical therapists treat many problems stemming from surgery, sports and work injuries, and even strokes and other disorders.

Please wear comfortable clothing that will allow the physical therapist to best observe the body region we will be treating. You may bring clothes (shorts, t-shirt etc.) and will be provided a private space to change into them. At times it may be necessary to observe more than one body region. In this case a gown will be provided for you.

Physical therapy coverage varies depending on your insurance policy. Our secretaries will verify your insurance benefits as a courtesy to you but the best way to understand your benefits is by you calling the number on the back of your insurance card.

Are you in my insurance network?

We do participate with many insurance providers. You can call the number on the back of your insurance card or check with one of our secretaries to see if we are in network with your provider.

What if I don’t have insurance?

NXT Level Physical Therapy does offer cash service payment plans. Check with one of our secretaries to see if this is an option for you.

Due to the nature of our practice, we are unable to always accommodate walk-ins and ask that all patients please make appointments ahead of time. You may call our clinic or make your appointment in person. If we have time on the schedule, we will try to get you in that day.

While a physician’s referral or prescription is not required for treatment it may be required by your insurance. We recommend that you contact your insurance to find out if they will require a prescription in order to cover your treatments.

You may cancel your appointment by calling the clinic you are being seen at. The receptionist and your physical therapist will then assist you in rescheduling your appointment. Please give our staff at least 24 hours or as much notice as possible.

To provide you with the most consistent care possible you will be working with the same physical therapist at each appointment. If for any reason a physical therapist will be gone the day you are scheduled for treatment, another physical therapist will have been fully briefed on your situation and treatment plan. If you should like to, for any reason, choose a specific therapist you absolutely may.

Your initial evaluation will usually last between 45 minutes to one hour, but sometimes could last longer if more problems/issues are identified. Each subsequent treatment visit will vary depending on your specific needs and progress. This will be determined by your physical therapist.
